Sibley Memorial Hospital is a non-profit, full service 318 bed acute care community hospital serving the Washington, DC area. Sibley offers medical, surgical, intensive care, obstetric, oncology, orthopaedic, skilled nursing inpatient services, and a state-of-the-art 24-hour Emergency Department. Sibley's campus, with its new patient tower, is also home to the Johns Hopkins Kimmel Cancer Center, the Johns Hopkins National Proton Therapy Center, Grand Oaks -- an assisted living residence -- a medical building with physician offices and an ambulatory surgery and imaging center.

SUMMARY:

The Case Management - Social Worker plays a pivotal role in evaluating the timeliness and appropriateness of patient care, coordinating discharge planning to ensure a seamless transition to post-hospital care, and striving to restore patients to optimal health and social adjustment. Additionally, they provide ancillary services to the Emergency and Psychiatry Departments by swiftly assessing patients with behavioral health complaints and collaborating with medical providers to determine the most suitable disposition and treatment for behavioral health patients and other vulnerable populations. Their essential duties also encompass identifying delays in care progression, recommending post-hospital care options, facilitating multidisciplinary meetings, documenting case coordination activities, accepting discharge planning orders, serving as a liaison to payers and external providers for post-hospital services, promoting smooth patient transitions, and identifying age-specific needs for protective services or post-hospital care, while referring cases regarding medical necessity to the Clinical Resource Management Committee.

MINIMUM REQUIREMENTS:

  • Licensed to practice as a Social Worker in the District of Columbia. Master’s degree in Social Work or sociology, gerontology, special education, rehabilitation counseling, or psychology from an accredited school.
  • Minimum of 2-5 years social work experience in a health care setting along with one or more years’ experience Case Coordination performing discharge planning from the inpatient or skilled facility setting and one or more years’ experience conducting inpatient utilization management and case coordination applying industry standard criteria (e.g., InterQual and M&R).
  • Current CPR Certification required.
