NURSE PRACTITIONER - SHARP

Johns Hopkins Community Physicians (JHCP) is looking for a Nurse Practitioner to join our Remington practice new SHARP model. SHARP (small high acuity/risk panel) is a Nurse Practitioner led small panel primary care program. Johns Hopkins SHARP providers co-manage a small panel of high-risk adult patients in a primary care practice, enhancing standard PCP care. The SHARP Nurse Practitioner collaborates with 3-5 established PCPs to provide enhanced, high touch, independent and collaborative primary care to a select group of high-risk patients. SHARP NP enjoy extended visits and significantly lower visit volumes than seen in standard primary care practices. Johns Hopkins Community Physicians serves Maryland, Virginia and Washington DC with over 40 locations. Our Providers and leaders are highly engaged and committed to teamwork. You’ll ­­­­­­­­­­­­­enjoy regular hours with night call less than once per month, allowing you ample time to enjoy all the activities that the area has to offer.

• JHCP practice at Baltimore, Maryland
• Full-time, 40 hours per week, 8am to 5pm
• Current NP Licensure in the State of Maryland required.
• CPR Certification required.
