Job Summary: The Clinical Pharmacy Specialist - Ambulatory Oncology, Provides advanced, evidenced-based pharmaceutical care and comprehensive medication management to ensure the safe and effective use of medications. Leads complex patient care activities, supports clinical guidelines and policy development, and contributes to quality-improvement initiatives that enhance safety and
outcomes. Functions with a high degree of autonomy and clinical judgment. Works collaboratively with prescribers and the multidisciplinary healthcare team to optimize patient outcomes and support evidence-based, cost-effective therapy. Participates in multidisciplinary committees and research initiatives. Provides education to patients, students, residents, pharmacists, and other healthcare practitioners.
Minimum Education and Experience:
- Graduation from an accredited College of Pharmacy resulting in B.S. or Pharm D. degree
Completion of a post-graduate year 1 (PGY1) ASHP accredited residency program or equivalent experience (3
years general practice) required
Completion of a post-graduate year 2 (PGY2) ASHP accredited residency program or equivalent experience (3
years specialized practice) required
• 5+ years of experience in clinical pharmacy practice required• Completion of an ASHP accredited Post-Graduate Year 1 Pharmacy Residency is equivalent to 3 years of
practice experience; completion of an ASHP accredited Post-Graduate Year 2 Pharmacy Residency is
equivalent to an additional 3 years of practice experience (6 years of experience total for PGY1 and PGY2
training)
• Pharmacist licensure in applicable state.
• Board Certification in applicable area required within 2 years of employment.
• BLS, ACLS and/or PALS may be required based on practice setting.
Shift: Full-Time, Day Shift, Monday - Friday 7:30am - 4pm; Ocassional On-call work required.
Salary Range: Minimum 61.55/hour - Maximum 101.51/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
