Social Worker
Join Our Team
As a Social Worker , you will play a vital role in supporting patients and families through some of their most challenging moments. Working collaboratively with physicians, case managers, and an interdisciplinary care team, you will provide compassionate psychosocial support, connect patients with critical resources, and help guide safe and effective transitions of care.

What You’ll Do
- Conduct psychosocial screenings and assessments for patients with identified social work needs
- Develop care plans to address emotional, behavioral, and discharge planning challenges
- Provide direct psychosocial support, transition planning, and care coordination
- Partner with the interdisciplinary team to promote safe, effective, and timely patient care
- Connect patients and families with community, regional, and national resources
- Support referrals related to guardianship, adoption, psychiatric care, abuse/neglect, IPV, end-of-life planning, and charity care
- Facilitate family meetings and contribute to interdisciplinary rounds
- Maintain clear, timely, and compliant documentation in the medical record
- Participate in quality improvement efforts, on-call coverage, supervision, and coverage support as needed
- May conduct home visits depending on assignment
What We’re Looking For
Education & Licensure
- Master of Social Work (MSW) required
- Maryland LMSW required
Skills & Strengths
- Strong interpersonal and relationship-building skills
- Ability to work collaboratively in a multidisciplinary team
- Deep knowledge of community resources and care coordination
- Strong clinical judgment and ability to synthesize patient information
- Excellent written and verbal communication skills
- Strong organizational skills and ability to prioritize effectively
- Proficiency with electronic medical records and standard office technology
Salary Range: Minimum 32.40/hour - Maximum 53.48/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
