SUMMARY

The Community Health Worker (CHW) provides outreach, care coordination, and patient education to help individuals overcome barriers to care and connect with the clinical and community resources they need to thrive. This role supports engagement across key populations, including emergency department, prenatal/postpartum, and post-discharge patients, while promoting continuity of care, self-management, and improved health outcomes.

In addition to core CHW responsibilities, this position performs more advanced duties, including mentoring junior staff and contributing to program improvement efforts.


ESSENTIAL FUNCTIONS

  • Conduct social needs screenings and intake assessments to identify barriers such as housing, food, transportation, insurance, and medication access.
  • Connect individuals to community resources, public benefits, and support programs; assist with applications and enrollment processes.
  • Coordinate care by communicating member needs and risks to clinical teams and facilitating referrals to medical, behavioral health, and social service providers.
  • Support appointment scheduling and help address logistical barriers, including transportation needs.
  • Conduct timely outreach and follow-up after discharge to support continuity of care and connection to recommended services.
  • Provide patient education, motivational interviewing, goal-setting support, and advocacy to encourage self-management and care plan adherence.
  • Document all interactions, referrals, and care activities accurately and timely in required systems.
  • Build and maintain effective relationships with community partners and referral sources.
  • Participate in program activities, quality improvement initiatives, and assigned projects.
  • Provide guidance and mentorship to junior CHWs and support consistent documentation and workflow practices.
  • Perform other duties as assigned.
  • Comply with all organizational policies and standards.
  • Use organizational resources efficiently and effectively.

QUALIFICATIONS

Education

  • High School Diploma/GED required
  • Associate’s degree preferred

Salary Range: Minimum 24.97/hour - Maximum 41.22/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
