Job Title: Community Health Worker

Position Summary

The Community Health Worker (CHW) supports patients in managing social and health-related needs by providing outreach, education, care coordination, and ongoing follow-up. Under supervision, the CHW works closely with interdisciplinary care teams, community partners, and patients to improve engagement, access to services, and health outcomes.

The CHW II may manage more complex patient needs, help train and mentor CHW I staff, support referral assignment, and lead special projects as assigned.

Key Responsibilities

  • Screen, enroll, and support patients in community-based programs
  • Provide patient education, mentorship, follow-up, and treatment plan reinforcement
  • Coordinate appointments, transportation, and community resources
  • Support patient engagement and help address barriers to care
  • Collaborate with physicians, nurses, care managers, behavioral health staff, and other CHWs
  • Build strong relationships with community organizations and internal teams
  • Assist with special projects, referral coordination, and training support as needed
  • Travel to patient homes, community sites, clinics, and meetings as required

Qualifications

  • High school diploma or equivalent required
  • Must be able to read, write, and speak English
  • Maryland CHW Certification required
  • Minimum of 2 years of experience in a social, community, healthcare, managed care, or medical office setting
  • For internal candidates, 1 year of CHW I experience may qualify as a promotional opportunity for CHW II

  • Excellent organization, follow-through, and problem-solving abilities
  • Ability to work independently and recognize when professional intervention is needed
  • Comfortable using a computer and entering data accurately
  • Compassionate, flexible, and effective in working with diverse populations

Additional Requirements

  • Reliable daily transportation for work-related travel required
  • If using a personal vehicle, valid driver’s license and proof of insurance are required
  • Completion of required training within 30 days of hire

Salary Range: Minimum 22.33/hour - Maximum 36.86/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
