SUMMARY
The Social Worker I provides direct psychosocial care, discharge planning, and care coordination in collaboration with patients, families, case managers, physicians, and the interdisciplinary team. Responsibilities include psychosocial assessments, identification of barriers to care, development of treatment and transition plans, patient/family education, resource referral, and clinical interventions to support safe, effective patient outcomes.
In the Community Psychiatry Program (CPP), the Social Worker I also provides direct psychotherapy, crisis intervention, grief counseling, abuse reporting support, and medication monitoring in coordination with the physician and care team.
ESSENTIAL FUNCTIONS
- Conduct psychosocial screenings and complete full assessments for patients requiring social work intervention.
- Develop and implement care plans and discharge/transition plans.
- Provide direct psychosocial support, psychotherapy, and crisis intervention as needed.
- Collaborate with the interdisciplinary team to support care coordination and optimize outcomes.
- Connect patients and families with community, legal, financial, and post-acute resources.
- Facilitate family meetings and support patient/family engagement and education.
- Assist with guardianship, adoption, psychiatric referral, abuse/neglect/IPV assessments, end-of-life planning, and charity care.
- Maintain timely, accurate documentation in accordance with departmental and regulatory standards.
- Participate in interdisciplinary rounds, quality improvement activities, and coverage as needed.
- Monitor medication effectiveness and report concerns or side effects to the physician when applicable.
QUALIFICATIONS
Education: MSW required
Licensure: Maryland LMSW required
Skills/Abilities:
- Strong interpersonal and collaborative skills
- Knowledge of community resources
- Ability to assess, synthesize, and document patient information effectively
- Excellent oral and written communication skills
- Proficiency with electronic health records and standard office technology
Salary Range: Minimum 32.40/hour - Maximum 53.48/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
